Terms of Service

1. Acceptance of Terms By accessing or using the Same Page Caregiving application or website ("Service"), you agree to be bound by these Terms of Service ("Terms"). If you do not agree to these Terms, please do not use the Service. 2. Description of Service Same Page Caregiving is a family care coordination platform designed to help family caregivers stay organized, communicate, and coordinate care for a loved one. Features may include but are not limited to: Shared care calendars and scheduling, Medication and task tracking, Team communication and updates, Document and health record storage, Caregiving resource links and educational videos, Family Care team management 3. Eligibility You must be at least 18 years of age to use this Service. By using the Service, you represent and warrant that you meet this requirement. Same Page Caregiving is intended for personal, non-commercial family caregiving use. 4. Account Registration To access certain features in the application, you must create an account. You agree to: Provide accurate and complete information during registration Keep your account credentials confidential and secure Notify us immediately of any unauthorized use of your account Be responsible for all activity that occurs under your account Same Page Caregiving reserves the right to suspend or terminate accounts that violate these Terms. 5. Care Team & Invited Users Same Page Caregiving allows you to invite family members, friends, and other caregivers to join your care team. By inviting others, you: Confirm you have their permission to share their contact information with us Understand that invited users will have access to information you choose to share within the platform based on permissions granted Accept responsibility for managing the appropriate access levels of your care team members 6. End User Agreement

Same Page Caregiving Effective Date: May 16, 2026 1. Agreement Overview This End User License Agreement ("Agreement") is a legal agreement between you ("User") and Same Page Caregiving ("Company") governing your use of the Same Page Caregiving mobile application and website ("Application"). By downloading, installing, or using the Application, you agree to be bound by this Agreement. 2. License Grant Subject to the terms of this Agreement, Same Page Caregiving grants you a: Limited — only for your personal caregiving use Non-exclusive — others may also be granted the same license Non-transferable — you may not transfer this license to another person Revocable — we may terminate this license if you violate this Agreement license to download and use the Application on devices you own or control. 3. Permitted Uses You may use the Application to: Coordinate care for a loved one with your personal care team Manage caregiving schedules, tasks, and medications Communicate and share updates with invited care team members Access caregiving resources and educational content provided within the Application 4. Restrictions You agree that you will not: Copy, modify, or distribute the Application or its content Reverse engineer, decompile, or attempt to extract the source code Rent, lease, lend, sell, or sublicense the Application to any third party Use the Application for any commercial purpose without written authorization Remove or alter any proprietary notices or labels on the Application Use the Application in any way that violates applicable local, state, or federal laws Attempt to gain unauthorized access to any servers or networks connected to the Application Use automated tools, bots, or scripts to access or interact with the Application 5. Ownership & Intellectual Property The Application and all of its content, features, and functionality are and will remain the exclusive property of Same Page Caregiving. This Agreement does not transfer any ownership rights to you. All rights not expressly granted are reserved by Same Page Caregiving. 6. User-Generated Content You retain ownership of the content you create within the Application. By entering content, you grant Same Page Caregiving a limited license to store, display, and transmit your content solely for the purpose of providing the Service to you and your care team. You represent and warrant that: You have the right to share any content you enter into the Application Your content does not violate any third-party rights or applicable laws You will not enter content that is harmful, false, or inappropriate 7. Updates & Changes to the Application Same Page Caregiving may from time to time: Release updates, patches, or new versions of the Application Add, modify, or remove features Temporarily suspend the Application for maintenance Continued use of the Application following updates constitutes acceptance of any revised terms. 8. Subscription & In-App Purchases Certain features require a paid subscription. All purchases are: Processed through the applicable app store (Apple App Store or Google Play) or our website Subject to the payment terms of the respective platform Non-refundable except as required by law or the applicable platform's refund policy You may cancel your subscription at any time through your account settings or the applicable app store. 9. Termination This Agreement is effective until terminated. Same Page Caregiving may terminate this Agreement and your access to the Application immediately if you: Violate any provision of this Agreement Engage in conduct harmful to other users or the Company Fail to pay applicable subscription fees Upon termination, you must cease all use of the Application and delete all copies from your devices. 10. Disclaimer of Warranties The Application is provided "as is" without warranty of any kind. Same Page Caregiving expressly disclaims all warranties, whether express, implied, or statutory, including but not limited to: Warranties of merchantability or fitness for a particular purpose Warranties that the Application will be uninterrupted or error-free Warranties regarding the accuracy or reliability of any content within the Application 11. Medical Disclaimer Same Page Caregiving is a care coordination tool only. It is not intended to replace professional medical advice, diagnosis, or treatment. Always consult a qualified healthcare provider for medical decisions. Do not delay seeking medical attention based on information within the Application. 12. Limitation of Liability To the maximum extent permitted by applicable law, Same Page Caregiving shall not be liable for: Any indirect, incidental, or consequential damages Loss of data, revenue, or profits Damages arising from your reliance on information within the Application Any interruption or cessation of the Service Our total liability to you shall not exceed the amount you paid for the Service in the twelve months preceding the claim. 13. Indemnification You agree to indemnify, defend, and hold harmless Same Page Caregiving and its officers, directors, employees, and agents from any claims, damages, or expenses — including reasonable attorney's fees — arising from: Your use of the Application Your violation of this Agreement Your violation of any third-party rights Any content you submit through the Application 14. Governing Law & Dispute Resolution This Agreement shall be governed by the laws of the Commonwealth of Kentucky. Any disputes shall be resolved through: Informal negotiation — you agree to first contact us to resolve any dispute informally Mediation — if informal resolution fails, disputes will be submitted to mediation in Jefferson County, Kentucky Binding arbitration — if mediation fails, disputes will be resolved through binding arbitration under applicable rules You waive any right to participate in a class action lawsuit against Same Page Caregiving. 15. Entire Agreement This Agreement, together with the Terms of Service and Privacy Policy, constitutes the entire agreement between you and Same Page Caregiving regarding the Application and supersedes all prior agreements. 16. Changes to This Agreement Same Page Caregiving reserves the right to modify this Agreement at any time. We will provide notice of material changes via email or in-app notification.
